Coordinate and evaluate the process changes made to comply with the clinical aspects of the Value Based Purchasing (VBP) measures for the region. Work with department leaders and clinical & support staff to improve processes based on VBP measures, that will result in improving patient care and improve efficiency.
Job Essentials :
Review all adult inpatient records to ensure Value Based Purchasing (VBP) clinical measures are met.
Electronically assess patients each day who are admitted through the Emergency Department and direct admits to units to capture all possible diagnoses that are measured as part of the VBP measures.
Follow up with appropriate persons for clarification of compliance with clinical VBP core measures and to share opportunities for improvement.
Maintain reports of compliance to be shared with appropriate persons for process improvement and provide education to staff on expectations of compliance.
Attend and participate in monthly service line meetings to provide follow up and process improvement in meeting our clinical VBP targets.
Lead the weekly VBP huddle and identify barriers and opportunities and garner support from leadership to overcome these obstacles to meeting our goals.
Partner with department and facility leaders to successfully meet the overall goals of VBP.
Populate the Electronic Discharge Tool (EDT) fields on admission and update as needed in preparation for discharge.
Consult with the Clinical Documentation Specialist nurses to identify the appropriate patient diagnosis or diagnoses.

MINIMUM REQUIREMENTS:
Bachelor’s degree
Two years of clinical experience in an acute inpatient setting.
Current professional license in clinical field.
Demonstrated success in collaborative work, data analysis and program development
Excellent verbal and written communication
Basic computer skills
Physical Requirements:
Speaking, hearing/listening, seeing, manual dexterity
PREFERRED QUALIFICATIONS:
Bachelor’s degree in a clinical field
RN or RRT experience
Leadership & mentoring experience in a clinical setting
Previous clinical quality experience
