Description
The Palliative Care Nurse Practitioner provides efficient and high quality consultation services to patients requiring a palliative care assessment and plan of care. They coordinate the plan of care with physicians, case managers, social workers, nurses and outside agencies as appropriate. Also supports research, quality improvement, education and outreach programs across the healthcare setting. This position will be responsible for development, coordination, evaluation and expansion of the palliative care program. NP will work with acute and ICU inpatient hospital services as part of an interdisciplinary team including: MD, RN, SW, and Chaplain to support patients and their families with advanced illnesses and/or acute life threatening injuries, including end of life. Duties also include symptom assessment and management and addressing advance directives while identifying patient centered goals of care. Palliative care is the integrated referral practice addressing the physical, emotional, psychosocial, and spiritual support of the patient and their family.

Shift: Mon-Fri 8am-5pm, some holiday and rotation on call
Department: IMC and LDS - slight amount of travel
Additional Details: Experience is preferred, but new graduate training is an option
Minimum Requirements
Current Utah State license as a Nurse Practitioner or license eligible if applying from out of state
Eligible for state Controlled Substance License II through V
Eligible for Federal DEA Certificate II through V

Preferred Qualifications
Certification from a recognized certifying organization such as American Nursing Credentialing Center (ANCC) or the American Academy of Nurse Practitioners.
National certification as Advanced Certified Hospice and Palliative Nurse (ACHPN)
Three or more years of previous clinical experience in nursing
Master's degree or higher
One or more years administrative experience including program evaluation, budget preparation, compliance and marketing
One or more years of experience in hospice or palliative care
One or more years of experience with research
