Intermountain Healthcare’s Clinical Programs and Services bring together expert teams dedicated to finding the treatments that have been proven to work for a given condition—and making sure these treatments are available throughout Intermountain’s service area.
For example, children admitted to Intermountain urban facilities with asthma have benefitted from our team approach. Doctors, nurses, and respiratory therapists work together to help patients and families learn about their asthma and how to control the symptoms. In addition, in one year, we increased by two-thirds the percentage of patients who went home with a detailed asthma action plan explaining how they should monitor and treat their asthma symptoms and when to contact their doctors. This tool helps patients better control asthma and avoid unnecessary hospitalizations or visits to emergency rooms.
Our Primary Children’s Medical Center hospitalist team first implemented these measures in 2008. That year, the hospital ranked tenth out of 31 freestanding children’s hospitals in the nation in the rate of asthma readmissions. Two years later, Primary Children’s was first in the nation, with the lowest rate of readmissions for asthma.
