How to Apply

An Intermountain Healthcare Foundation grant application packet has several required pieces. Your application will not be considered if any of the information is missing from your application.

The grant application you submit will be copied and distributed to the Foundation Board for review, so it is important that it be clear and easy-to-read!

Program Areas

The Foundation is focused on two areas of healthcare for low-income families and individuals. These categories are listed below in order of importance:

1. Direct Healthcare Services
2. Healthcare Support Services

Your Application

Applying agencies should be prepared to receive a site visit from a Board or staff member following receipt of the application but prior to the Board meeting. Eligibility Organizations that receive a grant will not be eligible to apply again for 18 months.

Info for Grantees

Grantees must be prepared to track and report program outcomes, as well as specific results that demonstrate measurable human impact. An outcome measures report is due at approximately eight months following the payment of the grant.
