- Take exit 276, merging left onto 500 East, in American Fork.
- Follow 500 East for about a mile.
- Turn right on State Street and follow for about a mile.
- Take a left onto 2000 West / 1100 East and follow for about a mile.
- Drive past the main entrance of American Fork Hospital.
- Turn right on 200 North.
- We are located in the second building on the left.
