Our highly trained rehabilitation team develops individualized treatment plans to meet the unique needs and requirements of each patient. Our treatment team and supportive services include physiatrists (rehab physicians), rehabilitation nurses, physical therapists, occupational therapists, speech/language therapists, social workers, audiologists, respiratory therapists and vocational counselors.

Chad Allred
Physical Therapist
Chad has been practicing physical therapy in the Utah County area since he graduated in 1999. He received his MBA from the University of Utah in 2011.
He enjoys treating a variety of orthopedic conditions. His interest in running and biking has led to becoming more familiar with the mechanics of the lower extremities and injury prevention. Chad is certified in Graston technique and Kinesiology taping. |

Jared Bumgardner
Audiologist
Jared Bumgardner graduated from the University of Utah with a doctorate of audiology. He works with many different populations ranging from adults to pediatrics. He has experience with many different areas of audiology, including basic audiometry, hearing aids, balance assessments and diagnosis of hearing loss in pediatric patients.
He began his career working in Salt Lake County at Intermountain Medical Center, LDS hospital, Alta View Hospital and Riverton Hospital. His expertise and test results will be a valuable tool to assist you in managing your patients with hearing, tinnitus and dizziness disorders. Jared also practices at:

Gordon Hines
Occupational TherapistGordon has a bachelor of science in occupational therapy from Eastern Michigan University. Gordon is the department manager and has been a practicing therapist for over 25 years.
He has obtained certification in hand therapy, ASTYM, GRASTON, and neuro developmental training. His primary practice interest is in the upper extremity injuries and disorders. Cecile Jacobson
Physical TherapistCecile graduated from San Jose State University in California. She has been a practicing therapist for 25 years and specializes in neuro-trauma rehabilitation, post-surgical orthopedic care and newborn intensive care. Cecile is based at American Fork Hospital, where she has worked for 16 years. |

John Linford
Speech-Language PathologistJohn received his bachelor's and master's degrees in speech language pathology from Brigham Young University and has 16 years of experience.
John specializes in aphasia, cognition, dysphagia (swallowing), modified barium swallow studies, pediatric modified barium swallow studies, patients with stroke, mini-stroke, traumatic brain injury, Parkinson's disease and other neuromuscular diseases. John also practices at:

Alexis Lockyer
Physical TherapistAlexis graduated with a bachelor of science from the College of Charleston in South Carolina and a master's in rehabilitative sciences-physical therapy from the Medical University of South Carolina.
Alexis has worked in a variety of treatment settings and enjoys treating many different types of physical problems including spine dysfunction, post operative issues and sports-related injuries. Alexis utilizes a hands-on approach to treating her patients. Alexis has been a licensed physical therapist for more than 10 years. She is certified in Graston technique and Kinesiology taping. |

Brad Middleton
AudiologistBrad Middleton, MS, CCC-A, has more than 15 years of professional experience in hearing assessment and hearing aid fitting. He received his bachelor's degree in speech and hearing science and his master's degree in audiology from the University of Utah.
Brad has been the staff audiologist at American Fork Hospital since 1995. Brad helped set up the Newborn Hearing Screening Program at the hospital. He is actively involved managing the Newborn Hearing Screening Program and provides diagnostic hearing evaluations for area doctors, including pediatricians, family practice, internal medicine and otolaryngology.
Brad enjoys fitting hearing aids and staying abreast on the newest technologies. He has a certificate of clinical competence from the American Speech-Language-Hearing Association and is a fellow of the American Academy of Audiology. Brad also practices at:

Shelley Shumway
Occupational TherapistShelley graduated from Mesa Arizona Apollo College as on occupational therapy assistant. Shelley specializes in post surgery rehab care for total hips replacement, fractures of the hip, total knees replacement, back surgery and neurological care.
Shelley works in both the inpatient and outpatient setting. She is based at American Fork Hospital. Shelley has been with Intermountain Healthcare for 19 years. |

Quintin Warnick
Physical TherapistQuintin Warnick, MPT, graduated from Loma Linda University with a master's degree in physical therapy. Since that time, he has been licensed and practicing in Utah, primarily Utah County.
