Sleep disorders can be related to many other health conditions, and we are here to help. The American Fork Hospital Sleep Center is dedicated to assisting primary care physicians and other specialists in the management of sleep disorders among their various patients.

Referrals

For doctors, nurses, and other office staff seeking to refer a patient for a sleep study or other test at the Sleep Center, please use the Sleep Center Referral Sheet.
