Knowing what to expect about an upcoming surgery can provide comfort to both you and your family. To help in your preparation, we have outlined information that allows you to take care of some important items before you come to the hospital.
Your surgeon's office will schedule your procedure with our scheduling staff. Our scheduling staff will contact your insurance and obtain the estimated cost and copay for your procedure. After the copay has been obtained, the scheduling staff will contact you with the amount that needs to be paid at the time of service.
After your physician has scheduled your procedure, our scheduling staff will contact you with a copay amount that needs to be paid at the time of service.
Financial assistance is available for qualifying patients.
Pick up and complete a surgery patient information packet from your doctor. If you didn't receive this packet from your doctor, you can pick one up at the hospital. Bring the completed packet with you on the morning of your procedure.
You will be contacted the day before your procedure (between 9am and 5pm) to be provided with the time you should arrive at the surgical center. If you have not heard from the surgical center by 6pm the day before your procedure please call (435) 207-4570 to receive your arrival time.
NOTE: Your arrival time will be earlier than your scheduled procedure time in order to complete the necessary preparations for the procedure.
