Candidate Application & Selection
Applicants must submit the following information by January 5th:
- Application

(Residency and Intermountain employee)
- Letter to prospective residents
- Three letters of recommendation

(Standardized form provided)
- Curriculum vitae
- College of pharmacy transcript
- Short essay describing your reasons for residency

Onsite Interviews
Selected applicants are requested to visit Salt Lake City for onsite interviews during January or February. The full day interview allows the candidates to meet with representatives from all practice areas.
Resident Matching Program of the American Society of Health-System Pharmacists
The Intermountain Pharmacy Practice Residency Program participates in the Resident Matching Program of the American Society of Health-System Pharmacists (ASHP) and applicants will be required to participate in the matching program as well.
