The Transitional Care & Rehabilitation Unit at Logan Regional Hospital provides individually structured rehabilitation services to patients who can benefit from daily physical, occupational or speech therapies or skilled nursing care.
Transitional Care & Rehabilitation recently expanded with a new hospice suite and a new physical therapy and rehabilitation area.
- Our large, spacious, private hospice suite provides a home-like atmosphere for hospice patients and their families. The suite will accommodate a large number of visitors.
- Our physical therapy and rehabilitation area with state-of-the art equipment and technology provides Transitional Care and Rehabilitation patients convenient access to physical, occupational, speech and recreational therapy.
What care is provided on the Transitional Care & Rehabilitation Unit?
Transitional Care & Physical Rehabilitation is a separately licensed part of the hospital called a skilled nursing facility (SNF) and is certified by Medicare. It is designed for a short-term stay to increase the patient’s strength and wellness to enable a better transition to home or another living center. Skilled care means healthcare services given when a patient needs skilled nursing or rehabilitation staff. The patient's needs are reassessed daily. Once goals have been met, the care team will work with the patient and his or her loved ones to develop a plan for safe discharge.
Transitional Care & Rehab patients have access to a physician when needed and 24-hour registered nursing care on the unit. The patient and family work with an interdisciplinary team designed for the patient's specific care plan. This team may include a physician; a physical, occupational or speech therapist; a social worker; a dietician; a pharmacist and a recreational therapist.
