Co-Pays

Co-payments are required at the time of service for both insured and uninsured patients. For uninsured patients, a minimum payment of $25 is required at the time of the visit. We accept cash, checks, and most major credit cards.

Insurance

We accept most insurances. However, it is your responsibility to verify with your insurance company that the doctor and the hospital are participating with your plan prior to receiving services.

Financial Assistance

Financial assistance is available to those who qualify. Applications can be obtained from our front office staff.

Lab Specimens

We send all of our blood draws and lab specimens to the McKay-Dee Laboratory for processing and billing. You are responsible for knowing whether your insurance plan requires that you utilize a lab other than McKay-Dee. If you fail to notify us in a timely manner, you will be responsible for any charges not covered by your insurance.
