My Health Care
Patients, 18 years or older, can register in person at participating clinics where they are receiving care. Registered patients can view test results, request appointments, renew prescriptions, view sections of their medical records, and communicate with their physician securely and privately online.
Our doctors split their time between teaching and patient care which means they typically do not have set schedules.
Patients may only schedule appointments 6-8 weeks in advance. Should you need an appointment further out than 8 weeks, you may ask one of our schedulers to contact you once the schedules become available.
If you are unable to make it to a scheduled appointment, we require a 24-hour notice. Failure to do so will result as a no-show status for that appointment. Established patients who fail to show to three consecutive appointments or five appointments within a year will need to see another doctor elsewhere. New patients who fail to show up to their new patient appointment will not be scheduled for future appointments.
We realize that situations may arise in which you need to be seen urgently. Our acute clinics are designed to see walk-patients.

Monday thru Friday 8:30 – 11:30 a.m.
*Space is limited. Arrive early to guarantee a spot.
A doctor is “on-call” at all times. The on-call doctor will assist you during afterhours in case of an emergency. To reach the on-call doctor, call (801) 387-2800. Calls to the on-call doctor should only be made in case of urgent situations.
We require two business days notice for refills. In order to expedite your request, please have your pharmacy contact us for the refill. Requests should be made via fax, (801) 387-5333.
