Process for Application to Join Medical Staff

Application for medical staff membership and privileges at Primary Children’s Medical Center is a two-part process.  You begin the process by sending an e-mail to the Medical Staff Coordinator at jane.renwick@imail.org or calling (801) 662-6520, to ask for a pre-application form.  After the pre-application form has been submitted and approved, a full application will be sent to you from the central office of Intermountain Healthcare, our parent organization.

The initial credentialing process usually takes 3 - 4 months, so please plan accordingly.
