Request an Appointment or Refer a Patient Electronically

If you are a patient and would like to request an appointment online please fill out the information on our request an appointment form. A scheduler will contact you shortly to schedule your appointment.

If you are a Physician then your office will be contacted within 1–2 business days to coordinate the appointment. If you are referring a patient to a specific hospital or department then please fill out the required information on our physician referral form.

Preparing for your visit - Medical Records

At least one week prior to your scheduled clinic visit, please be sure to have your or your child’s medical records faxed or mailed to the Cardiology Medical Records Office from their referring physician’s office. This allows enough time for your records to be processed and reviewed in advance by your Cardiologist.
