Billing & Insurance

Full payment is appreciated the day of service, unless there is a pre-arranged agreement through your employer. We accept personal checks, cash, and all major credit cards.

Our administrative staff can give you the necessary information to file an insurance claim. Because LiVe Well Center procedures are preventive in nature, the costs of the procedures may not be covered by insurance. If you expect your insurance company to cover part or all of your examination costs, we highly recommend that you check with your insurance company before scheduling your appointment. It is your responsibility to know what your insurance will and will not cover.

We are not authorized to bill Medicare. If you are a Medicare patient, we will ask you to sign a "Medicare Waiver," and we appreciate full payment the day of your assessment.

This clinic is part of the Intermountain Medical Group, which is owned and operated by Intermountain Healthcare.
