Our Team Approach
The Personalized Care team consists of Internal Medicine physicians, a Palliative Care physician, a registered nurse, psychiatric APRN, pharmacist, therapist and other caregivers who will work with patients.
In addition to all the resources of a traditional primary care clinic, the Personalized Care clinic offers 60 minute initial appointments and 30-45 minute follow-up appointments to allow time to assist patients with complex conditions, mental health concerns, and medication challenges.
Coordination of Specialists
Our providers lead coordination with all the specialists a patient may see (e.g., cardiologist, endocrinologist, physical therapist, etc.). If a patient needs to go to the hospital, our providers coordinate recommended follow-up care with hospital specialists, too.
The Personalized Care Clinic employs a full-time pharmacist available to meet with patients, manage multiple medications, coordinate refills, and educate the patient how to take their medications properly.
Our specialists focus on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis. The goal is to improve quality of life for both the patient and their family.
