Please bring your filled out paperwork with you to your first appointment. If you do not have your paperwork on the date of your appointment, you may be asked to reschedule. If you need to reschedule your appointment, please notify us 48 hours in advance so that we may be able to fill your slot with others on the waiting list. Thank you.
