This award considers the rate of respondents who answered "excellent" to the question "Overall, how would you rate your experience with this visit?" as part of a standardized survey. It is given by SelectHealth and Intermountain Healthcare to the top 10% of clinics across the state of Utah.

This award's criteria consist of four major components: adoption of health information technology, regular use of population care management methods, participation in a data-reporting quality initiative, and exemplary performance on nationally-recognized clinical outcome measures.
