Who are we?
The Immunization Clinic at American Fork Hospital is a satellite clinic of The Utah County Health Department - Division of Family and Personal Health. Our location in north Utah County enables us to serve more families in a convenient manner.

What do we offer?
We provide all child and adult immunizations to non-insured or underinsured individuals as well as to patients with select health insurances. We offer the following vaccines:
  • DTAP, DT, TD
  • Flu
  • Polio
  • Hepatitis A, Hepatitis B and the combined Hep A/B
  • HIB
  • MMR
  • Pneumonia 7 (Prevnar) (infant)
  • Pneumonia 23 (adult)
  • Tuberculosis Skin Test
  • Meningococcal
  • Typhoid
Insurance and payment options
Children who are eligible for public funded vaccine programs will not be refused immunization because of inability to pay. The requirements for public funded vaccine programs are as follows:
  1. 0 thru 18 yrs of age
  2. On Medicaid
  3. Alaskan Native/American Indian
  4. Without Insurance
  5. Underinsured Clients who are insured without immunization coverage
All other children with insurance and adults will be charged for the vaccine.

We have a contract with certain insurance companies so we can bill them directly. They are SelectHealth, PEHP, United Health Care and Altius. We can also bill Medicare part B for Flu and Pneumonia vaccines.
