Transitional Care Center

The Transitional Care Center (TCC) at American Fork Hospital provides individually planned nursing and rehabilitation services for patients who can benefit from additional intensive therapy before leaving the hospital. This service benefits patients who no longer need acute care, but still require skilled nursing to assist them in their return to normal activities. The American Fork Hospital TCC is a licensed skilled nursing facility.

Admission criteria
TCC patients must meet the following criteria:
  • Require skilled nursing care as defined by Medicare.
  • Require daily physical therapy.
  • Length of stay may not exceed 30 days.
Interdisciplinary team
At the core of hospital care is a team of nurses working with the patient's personal physician and supporting departments. The interdisciplinary team for TCC patients consists of representatives from nursing, recreation therapy, physical therapy, occupational therapy, speech/language pathology, social services, pharmacy and respiratory therapy. All patients in TCC receive around-the-clock care from registered nurses, licensed practical nurses and certified nurses aides. The health care team is happy to discuss a patient's care plan with the patient and his or her family members.

Therapy
Therapies and evaluations may be provided in the center or in a specific department within the hospital, depending on the patient's needs. A therapeutic recreation specialist organizes and supervises an activity program that helps prepare the patient to return to a full and active lifestyle.

The TCC nursing team is supplemented by trained and caring social workers ready to assist with personal or family-related emotional challenges. Making arrangements for proper follow-up care enhances continued recovery after discharge from the unit.

Patient rooms
Private rooms are decorated with a home-like atmosphere to help patients feel as comfortable as possible. The TCC also features a dining room as well as a commons room, providing a friendly gathering place for all patients. Other amenities include in-room television and bedside phone, a hair care room as well as fully automatic and adjustable hospital beds for maximum comfort.

Visitors
Visitors of all ages are encouraged to spend time with the patients. Throughout the year, the TCC hosts parties to celebrate special events. These celebrations are a wonderful way to enhance recovery. In most cases, patients will be asked to have personal clothing brought to the unit, so patients are able to leave "hospital gowns" and return to their normal attire. Other personal items that will not disrupt the rehabilitation process are also encouraged.
