Advance Directives
You have the right to make decisions about medical care you receive while you are in the hospital. Circumstances may arise, however, that could prevent you from making those decisions for yourself.
Advance directives provide instruction that helps establish medical care treatment based on your wishes. An advance directive gives legal authority to another person to make health care decisions on your behalf, when you are unable to do so because of illness or injury.
You can change or revoke completed advance directive documents at any time.

Living Will
A living will is a document in which you choose the kind of life-prolonging medical care you wish to receive, if you become terminally ill or unable to make decisions about your care.
Medical Treatment Plan
A medical treatment plan is a written summary that describes what medical care and treatment is to be provided or withheld, when you cannot be consulted. The medical treatment plan is agreed upon by you and your physician, or by your family and your physician.
Special Power of Attorney
A special power of attorney is a signed, dated, witnessed and notarized legal document, in which you appoint an individual to make decisions regarding your medical care if you are unable to make them for yourself. You may also include specific instructions about treatments you may or may not wish to receive.
