Physicals

- Level one. This is a nursing assessment by an RN, and includes the following: Health history (current medications, tetanus status, allergies), height, weight, blood pressure, pulse, respirations, vision screening.
- Level two. This includes all of level one, plus: Physician exam and dip-stick urinalysis (baseline screening for 10 potential urine abnormalities).
- Level three. This includes all of level one, plus: Lifestyle/health risk questionnaire, cholesterol/HDL/ratio (checks for heart disease), blood sugar test (checks for possible diabetes), and percent body fat.
- Level four. This includes all of levels one and two, plus: Assessment of physical limitations, filing/storage of exam results for at least one year, referrals and follow-up if indicated.
- Other screening options with physical: Hearing screenings, chest x-ray, pulmonary function testing, urinalysis, urine drug screening, EKG, submax bike test, treadmill test.
Drug Screenings- DOT, industrial, and private drug screenings. Includes a five-drug panel to detect the most common sources of workplace drug non-compliance. Includes PCPs, amphetamines, cocaine, opiates, and marijuana. No chance of false positive results due to legally consumed drugs, with the exception of opiates and amphetamines. Expanded panel available also tests for methaqualone, propoxypene, benzodiazepines, methadone, and barbituates. On-site collection available for volumes of 15 or more employees scheduled concurrently.
- Additional options include: Breath alcohol testing, random screens, and consultations with an MRO-certified physician. Logan Regional Hospital is also a qualified DOT drug collection site, with referral available to the client's lab of choice for testing. In addition, consortia pool group for small or owner-operated trucking companies is available.
NOTE: State law requires confirmation by GCMS on all positive drug screens. Positive drug screens referred to certified lab for confirmation.
