NOTICE: ALL INTERMOUNTAIN RESEARCHERS
As of April 26, 2006, the Intermountain Office of Research is requesting that all new applications for human subjects and data research be submitted online through the Research Management System (RMS).
The following URL will allow users to access RMS from within Intermountain's Intranet and externally from any computer with access to the Internet:

Users must have an Intermountain LDAP login and password to access the applicationinternally and externally. LDAP logins may be requested by contacting the Intermountain Healthcare Office of Research:

The following forms are provided for conducting research at any Intermountain Healthcare facility. Complete and submit the proper forms to the appropriate IRB.
NOTE: All forms are fillable Word documents. Please fill out forms completely, utilizing the gray shaded areas to enter text. Incomplete forms may be returned and the application process delayed.
