    NOTICE: ALL INTERMOUNTAIN RESEARCHERS

    As of April 26, 2006, the Intermountain Office of Research is requesting that all new applications for human subjects and data research be submitted online through the Research Management System (RMS).

    The following URL will allow users to access RMS from within Intermountain's Intranet and externally from any computer with access to the Internet:

    Users must have an Intermountain LDAP login and password to access the applicationinternally and externally. LDAP logins may be requested by contacting the Intermountain Healthcare Office of Research:

    Initial Application Review
    The following forms are associated with an initial submission of an application for research at IHC. Please consult the Initial Review Checklist to familiarize yourself with the required materials before starting work on the necessary applications. Also, please pay attention to the administrative responsibilities listed on the checklist and make sure these requirements are met before submitting the application materials to the IRB.

    Please submit the completed checklist with the required IRB materials to ensure that all materials have been properly submitted to the IRB.

    Note: All forms are fillable Word documents. Please fill out forms completely, utilizing the gray shaded areas to enter text. Incomplete forms may be returned and the application process delayed.
