Dispute Resolution Agreement

Today, your doctor has ways to help you if you're hurt or sick that he or she didn't have just a few years ago. At the same time, healthcare still isn't perfect and mistakes can occur. It's good to know that doctors and hospitals find and fix most mistakes. But if you were injured from the care your doctor or the hospital provided, Intermountain offers helps to resolve concerns about medical care.

When you visit a physician who is part of the Intermountain Medical Group, you will be asked if you would like to voluntarily sign a Dispute Resolution Agreement. The agreement outlines how concerns about your medical care will be resolved.


The Dispute Resolution Agreement is completely voluntary. You do not have to sign the form to see a doctor or receive medical care. If you would like, you can print a sample copy of the agreement to review. Or, if you receive the form during a visit to a medical facility, you may take it home with you to review. There is no requirement or obligation to sign the form.
