What We Fund

We provide donations to community-based organizations - 501(c)(3) nonprofits and public agencies - in Utah, Southeast Idaho (Cassia County, Minidoka County), and Nevada (Clark County).

The Community Partner Fund aims to support projects, programs, and services that align with Intermountain Health’s identified health priorities for 2023-2025. These priorities include:

  • Improve Mental Well-Being
    • Reduce suicide deaths
    • Reduce frequent mental distress, including anxiety and depression
    • Decrease opioid misuse
  • Improve Chronic and Avoidable Health Outcomes
    • Prevent high blood pressure
    • Prevent diabetes
    • Increase immunization rates
  • Address Social Determinants of Health
    • Improve stable and quality housing through wraparound and support services
    • Improve nutrition security
  • Increase Access
    • Comprehensive medical, dental, vision, and/or behavioral health care

Donations are generally made once a year in amounts less than $5,000.

What We Do Not Fund

  • Individuals
  • “Out of Area” organizations
  • Event sponsorships
  • Marketing activities

Requesting a Donation

You can submit a request at any time during the year, subject to fund availability. Notifications of any approved or declined donations will be provided via email. Organizations are eligible for funding once per calendar year.

When requesting funding, please provide the following documentation:

  • Copy of the organization’s 501(c)(3 ) designation letter or exemption certificate for public service agencies.
  • Copy of the organization’s W-9
  • Intermountain Health Supplier Form
