At the Park City Hospital’s LiVe Well Center, we recommend starting your new year off with a lifestyle consultation to get a baseline of your overall health and wellness. A lifestyle consultation can help educate and equip you with the knowledge need to stay healthy and fight diseases.
With a lifestyle consultation we take a look at your complete daily routine; what is your sleep schedule, eating habits, physical activity, stress levels, and foot intake. We will also take into account your family health history in order to address any future health concerns. Once we have your big picture outlined, we work with you to develop a personal action plan that you can succeed with. Your action plan will include: setting specific “SMART” goals, addressing barriers that would prevent you from reaching your goals and strategies to overcome them, arranging follow up visits, creating an accountability structure with family, friends, and the LiVe Well Center team, and finally how to share your plan with your support group.
