Utah Valley Sports Medicine Fellowship
Utah Valley Sports Medicine participates in the National Residency Matching Program. Documents that must be submitted directly to the program may be sent to the fellowship coordinator.
All applications will be processed using the ERAS system. ERAS required application components:
U.S. citizens or permanent residents only. We do not sponsor visas.
You will need to register for the match with the National Resident Matching Program (NRMP).
All application materials must be received by October 1.
If you are a second or third year resident, or fourth year medical student interested in exploring our program by completing a rotation with us, you can apply through the Resident and Medical Student Rotation Request Form.
