Use the Symptom Checker to Qualify for a COVID-19 Test

Step One: Click the chat bot in the bottom right corner of your screen
Click here to see if you qualify for a test.

See if you qualify for a COVID-19 test by accessing the online symptom checker on this page. If you qualify, you will be directed to reserve an arrival window online at an Intermountain community testing location. You will receive a confirmation email with your arrival window time, date, and location attached – please do not go to a testing site without having an arrival window reserved.

If a healthcare provider has ordered you a test with Intermountain:

  • You do not need to use the symptom checker.
  • Once you receive a text or email from Intermountain with your visit ID, go to your designated testing location. An arrival window is not needed.
  • Paper orders or images of orders are not accepted at testing locations.
  • If you have questions – or have not received a text – please see the FAQs below.

If you are having a procedure or delivering a baby at an Intermountain facility:

  • You do not need to use the symptom checker.
  • Once you receive a text or email from Intermountain with your visit ID, go to a drop-off location.
  • Paper orders or images of orders are not accepted at testing locations.
  • If you have questions – or have not received a text – please see the FAQs below.
