This was my second visit with Joan Shelley the NP and because I wanted to be Sure that the NP understood why I am here I completely repeated and described again what I expressed on my first visit. I think the repeat description was understood by Joan Shelley as a problem with my cognitive abilities which was not the case at all. I merely wanted to be sure the problem was completely understood by this provider.As a result, she ordered the nurse assistant to perform a cog test on me, which was in my opinion a mistake. This Nurse assistant has a slight accent and she speaks very quickly which for me was making It hard to follow the verbal instructions and phrases I was to repeat. As a result the impression would be that I was not OK. On my next discussion with Joan Shelley it will be discussed, at which time I will request that it will be removed from my records.
