Process for Application to Join Medical Staff

Application for medical staff membership and/or privileges at Primary Children’s Hospital is a two-part process. You begin the process by sending an e-mail or calling the Medical Staff Coordinator to ask for a pre-application form. After the pre-application form has been submitted and approved, a full application will be sent to you from the central office of Intermountain Healthcare, our parent organization.

The initial credentialing process usually takes 4 - 6 months, so please plan accordingly.
