How to Prepare For...

Before Your Appointment

At least one week prior to your scheduled clinic visit, please be sure to have your or your child’s medical records faxed or mailed to the Pediatric Cardiology office from their referring physician’s office. This allows enough time for your records to be processed and reviewed in advance by your cardiologist.

Records can be mailed to:
Primary Children’s Outpatient Services
Pediatric Cardiology
81 N. Mario Capecchi Drive, 4th Floor
Salt Lake City, Utah 84113

Records can be faxed to (801) 213-7778

You may have more than one bill associated with your visit to the Heart Center. Services provided by the facility will be billed by Intermountain Healthcare. Services provided by your cardiologist will be billed separately by University Medical Billing. For example, if your clinic visit includes an echocardiogram, it will generate a technical fee that will be billed by the hospital. The cardiologist’s professional interpretation of the echocardiogram will be billed by University Medical Billing.

Patients seen at the Cardiology Clinic at Primary Children’s Riverton facility will receive one bill for both the professional component and technical charges from University Medical Billing.

Your insurance may or may not cover clinic services. Check with your insurance provider to confirm coverage.
