  • Hold a current GPA of 3.5 or higher
  • Plan to pursue a career in healthcare
  • Be actively involved in community service

  • Completed application
  • Resume
  • 300-500 word essay

The essay should provide details on the student’s experience with community service as well as plans for pursuing a career in healthcare, and how they would use the scholarship award if selected.

Sanpete Valley Hospital
Attn: Debbie Robinett
1100 S. Medical Drive
Mt. Pleasant UT 84647

Applications submitted via mail must be postmarked by April 21 deadline.

Selection

Representatives from Sanpete Valley Hospital will review the applicants and select three students to each receive a $1,000 scholarship award. Selection will be based on the student’s application, transcript, resume, and essay.

Award Process

Scholarship recipients will need to provide proof of college registration to Sanpete Valley Hospital and complete applicable tax forms in order to receive the award. Sanpete Valley Hospital will then make the $1,000 donation towards the student’s tuition or other schooling expenses (please allow at least four weeks processing time from when the forms are submitted to the time that the award check is received). Scholarship awards will be held for up to three years for those recipient(s) who will not be attending college for the following school year(s).
