Personalized Primary Care is a patient-centered model of healthcare delivery being implemented by the Intermountain Medical Group. This model is guided by Intermountain Healthcare’s Mission, Vision, and Values and is based on the National Committee on Quality Assurance 2011 Patient-Centered Medical Home Standards.

A Medical Home comprises a team of health professionals who work together to help patients get healthy, stay healthy, and get the right care and services at the right time and in the right place. A Personalized Primary Care team can include a doctor, an Advanced Practice Clinician (NP or PA), a Care Manager, a Health Advocate, Mental Health Partners, and other clinical and office staff.
