The Digital Front Door (DFD) is Intermountain Healthcare’s new integrated digital experience that empowers you to take an active role in managing your health, from booking an appointment to paying your bill. We are currently piloting this new experience at three Intermountain clinic locations: Layton Family Practice, Memorial Internal Medicine, and West Valley Family Practice.

  • The App: Download the DFD mobile app from the App Store or Google Play.
  • The Web: Access the DFD web platform here. Bookmark the page so it's easy to visit again.

A Few Items to Keep in Mind

Overall
  • The Digital Front Door is currently supported for iOS 9 and up and Android 4.1 and up.
  • The Digital Front Door is currently only available in English. We’re actively working to make it available in other languages in 2020.
  • If you already have a My Health account, you can use your existing My Health username and password to log in to the Digital Front Door app and web platforms.
  • Pilot participants must be comfortable with viewing personal health information, including weight, in the platforms.
Feature Specific
  • Account Creation: Currently, only existing Intermountain patients or SelectHealth members (e.g., users who know their Patient ID, SelectHealth Subscriber Number, Medicaid ID, or SH Insurance ID for Rx-only plans) can register for the Digital Front Door. Please check back later for expanded access.
  • Appointment Booking: Online appointment booking is only available at the three pilot clinic locations: Layton Family Practice, Memorial Internal Medicine, and West Valley Family Practice.
  • Get Care Now: The Connect Care feature within Get Care Now works best with binary sex options (Male/Female). If a pilot participant has a nonbinary sex listed in their account, their experience may be impacted. We are actively working to resolve this issue.
  • If you experience difficulties accessing Connect Care via DFD, please download the Connect Care app by searching “Intermountain Connect Care” in the app store.
