You are more than just a patient, and Intermountain Clinics are more than just doctor's offices.

We are bringing Intermountain Personalized Primary Care (PPC), a new patient-based model of care, to Intermountain Clinics across Utah. The Personalized Primary Care model recognizes each patient as an individual with unique healthcare needs and provides an enhanced range of services.

Our model emphasizes preventive care, patient education, and empowering patients to better maintain their health. Additionally, we encourage our Personalized Primary Care clinics to focus on chronic conditions such as diabetes, hypertension, depression, attention deficit disorder, asthma, and coronary artery disease.  

Each participating Personalized Primary Care clinic employs healthcare professionals, led by your doctor, who help you maintain good health and manage your medical conditions.

Personalized Primary Care Team

Your Personalized Primary Care team includes:

  • Your Physician, who leads your team of caregivers, is your trusted advisor, answers your questions, and builds a long-term relationship with you.
  • A Health Advocate who reviews your chart and checks in with you at each visit to answer your questions and connect you with the resources you need
  • A Care Manager who works directly with you and other health specialists to manage a chronic disease or other health needs
  • Clinical and Office Staff, including medical assistants, lab and imaging staff, Patient Service Representatives, and Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants). They schedule appointments, gather information for your visits, draw labs, and take imaging studies. They also work with insurance providers and organize your records.
  • Mental Health Partners may also be available to help connect you with therapists, psychologists, or other mental health practitioners.  

In Line With National Initiatives

Intermountain Personalized Primary Care is based on "medical home" models of care, such as the model developed by the National Committee for Quality Assurance (NCQA) along with the American Academy of Pediatrics, the American Academy of Family Practice, the American College of Physicians, and the American Osteopathic Association. This model places the doctor's office at the core of an enhanced range of patient services, including better care coordination and wellness and preventive services.

Standards associated with national and local healthcare improvement models include:

  • Improved patient access and communication
  • Care management and coordination
  • Encouraging patient autonomy
  • Electronic prescriptions or e-Prescribing
  • Reporting the performance of clinics and doctors 
  • Health information technology usage
