Lifestyle Oriented Primary Care

Lifestyle oriented primary care is a "lifestyle-first" approach to optimizing health and wellness and managing chronic conditions. LiVe Well Center physicians and advanced practice providers focus on physical activity, nutrition, sleep, stress management, and environmental exposures as strategies to improve health and quality of life over time; while prescribing medications and additional treatments and tests as needed.

Living Well with Chronic Conditions

Better Choices Better Health® is an online program designed to help you learn how to effectively self-manage a chronic condition. The program occurs 2 hours per week for 6 weeks and is available online at no charge.

Participants’ self-reported measures indicate reductions in health distress, pain and disability; an increase in aerobic and strength exercises; and reductions in health care utilization, including physician visits, emergency room visits and days spent in hospital.

Living Well with Chronic Pain

The Living Well with Chronic Pain Self-Management Program is a 6 week evidence-based program, originally developed at Stanford University, for people with long-term pain.  Participants are taught how pain is processed in the brain and how factors such as depression, anxiety, stress and other emotions affect our brain’s interpretation of pain.  Participants also:
  • Learn how decision making and problem solving skills can help them to adjust and adapt.
  • Understand why nutrition and physical activity relate to pain management.
  • Practice skills like open communication and coping with difficult emotions.
  • Participate in breathing exercises, gentle stretching activities and other distraction techniques.
