Who We Are

We are a group of caregivers and families that work together to promote patient- and family-centered healthcare. Family members are parents and family members who care for children who have been Heart Center patients (either inpatient or outpatient) at Primary Children’s. Caregivers involved include the Heart Center Co-Directors, physicians, registered nurses, administration, and other disciplines.

What We Do

Together, Caregivers and families meet to facilitate ideas and solutions on how we best deliver exception care to our heart population. We strive to be solution focused and really look to family members to offer insight based on experience, while caregivers provide insight from professional backgrounds. Together, we hope results and changes can be achieved.

Our HC-FAC meets quarterly in-person and virtually as well. Meetings are 1.5 hours with a dinner served. Agendas will be prepared by FAC Coordinator and disseminated.

How to Join

You are eligible to join the HC-FAC if you have a child who has received heart care at a Primary Children's facility. Please fill out the application linked below before September 1st, 2022. Candidate selection will occur in October 2022.
