Intermountain Health focuses on innovation and healthcare transformation and is committed to delivering better care and improving outcomes at lower costs.
The pursuit led to a partnership with Omada Health, a virtual, chronic disease prevention and management provider that works with healthcare systems to improve access, outcomes, and compliance with chronic disease prevention and management.
The initial partnership provided access to some of Omada’s programs for Intermountain’s patients and caregivers.
Omada's diabetes prevention program was rolled out as a benefit to Intermountain Health employees in 2019.
Dr. JP Valin, Chief Clinical Officer at Intermountain Health said, “There are 96 million adults in the U.S. diagnosed with prediabetes, and 37.3 million diagnosed with diabetes (90-95% of which is type 2 diabetes).”
“Being able to support Intermountain’s community and see its [Omada’s] participants experience positive change is what drives us to do what we do. Intermountain’s focus on implementing virtual care for better outcomes aligns with our commitment of providing virtual-first care programs that support best possible patient outcomes,” said Dr. Valin.
The Community Health team also wanted to open the diabetes prevention program to community members, as part of our priority to prevent chronic disease.
Health Promotion and Wellness Director, Jessica Shields, and Community Health Program Manager, Priscilla Martinez, have been using innovative approaches to fill the 200 spots open to community members.
For patients of Intermountain Health, their electronic medical record data easily reveals if they have lab confirmed prediabetes and are an eligible candidate for Omada, but it’s not as simple for underinsured populations.
“We’re taking a ‘just-in-time' enrollment approach. We need to reach these populations where they are and get them access quickly," said Shields.
Omada has created flyers that direct community members to a landing page specifically designed to screen people for prediabetes.
If people score a 5 or higher indicating they are at high risk, they can enroll in Omada the same day.
Martinez provided the flyers to several Utah health departments and community organizations.
Through their efforts they’ve filled more than 100 spots.
The community participants have:
- Engaged with the platform 28.5 times/week
- A lesson completion rate of 83.5% (Omada’s benchmark is 79%)
- 32% are achieving at least 5% weight loss at week 52% (Omada’s benchmark is 29%)
- Participant satisfaction rate of 91%
The Community Health team have also pushed to get options for the Spanish-speaking community so we now have access to Omada’s has bilingual offerings.
