The Intermountain Community Care Foundation provided funding support for the Food Rx pilot program lead by Creek Valley Health Clinic and Short Creek Dream Center over two years.
The Food Rx program aims to increase access to healthy foods for rural residents that have been diagnosed with chronic disease.
This pilot program highlights how Food Rx programs can address food insecurity and improve health outcomes in rural, underserved, and impoverished communities.
For the first year of funding, the Food Rx program reported that 80% of patients diagnosed with diabetes reported adequate blood sugar control, 61% of patients diagnosed with hypertension had adequate blood pressure control and 3% of patients with previously diagnosed depression (12 months prior) now experience depression remission.
One patient who responded to an anonymous survey said, "I went to Creek Valley for a follow-up, and they had no idea how tight we were with money. My doctor gave me a nutrition cookbook about vegetable counts and fiber-fueled eating, which I loved so much! We mostly eat what we grow in our garden each year. It was perfect for our family. Not only that, but they sent us out the door with a $20 gift card to Bees for fruits and veggies. It was a real boon during a difficult time.
