Utah Valley Sports Medicine participates in the National Residency Matching Program. Documents that must be submitted directly to the program may be sent to the fellowship coordinator.

Application Requirements

All applications will be processed using the ERAS system. ERAS required application components:

  • MyERAS Application
  • Personal statement
  • Three letters of recommendation: Preferable that at least one be from a sports medicine physician
  • Medical school transcript
  • USMLE/COMLEX transcripts
  • Photograph
  • Sports Medicine Experiences, if applicable. Noteworthy experiences include:
    • Sports Medicine Shadowing (dates, type, location, instructor)
    • Sports Medicine Rotation (dates, type, location, instructor)
    • Sports Medicine Coverage (games, events, training room, other)
    • Attended Sports Medicine Conferences
    • Presented at Sports Medicine Conferences

Submit Directly to UVSM Fellowship

Email the following documents to the fellowship coordinator at Angela.Herbert@imail.org

  • Signed AMSSM Code of Ethics for the Match
  • A copy of the title page from your publications, if applicable
  • A copy of the program of any listed sports medicine conference presentations given, if applicable

Primary Specialties

Physicians who have completed residency training in Family Medicine, Emergency Medicine, Internal Medicine, Pediatric Medicine, Internal Medicine / Pediatric Medicine (Med-peds), or Physical Medicine and Rehabilitation will be considered. 

Visas

U.S. citizens or permanent residents only. We do not sponsor visas.

Register with NRMP

You will need to register for the match with the National Resident Matching Program (NRMP).

Application Deadline

All application materials must be received by October 1.

Medical Student and Resident Rotations

Due to the COVID-19 situation, we are not currently accepting medical student rotators. A limited number of fellowship applicant audition rotations may be possible. Please reach out to the fellowship coordinator if you are interested in scheduling an audition rotation.
