Have you ever wondered what would happen if you were to suffer a serious car accident or sudden illness and not be able to speak for yourself? 

  • Who would make crucial decisions for you? 
  • How would they know what you wanted?
  • How can you plan to be sure you get the care and medical attention you would want in an emergency? 

Advance Care Planning (ACP) is thinking and talking about future healthcare decisions long before a sudden, emergent event, like a car accident happens and you are unable to make your own decisions. The results of the conversation are communicated on an advance directive.

An Advance Directive is a legal form used to identify who you would want to make medical decisions for you if you were unable to do so.  It also allows you to inform healthcare providers of what treatment you want or don't want.  It is not an order; only a guide to treatment that may or may not be wanted. 

In this workshop, you will:

  • Learn more about the purpose of Advance Care Planning (ACP)
  • Learn about what questions to reflect on and how to align your values with what matters most to you
  • Learn what Advance Care Planning options you have, including choosing a healthcare agent (a person who makes decisions for you when you are unable to)
  • Learn what happens without Advance Care Planning
  • Learn what steps to take for completing Advance Care Planning
