Birth certificates and Social Security cards

A birth certificate is an important legal document that you will need at various times during your child’s life. It is used when you register your child for school and when you apply for a passport or for social security benefits.

The Birth Certificate Process

While you are in the hospital, you will fill out, sign and date the State of Colorado Birth Certificate Worksheet provided to you. A copy of the application is given to you at the time the Birth Certificate is completed. Once all information is complete, the information is sent to the State of Colorado the day after discharge and is ready within 48 hours of being released.

To obtain your copy of the Birth Certificate, contact:

Colorado Department of Health and Environment
Vital Records Section
4300 Cherry Creek Drive South
Denver, CO 80246-1530
Tel: 303-692-2200

There are many additional vital record offices. You can call to see if there is an office closer to your home to pick up a Birth Certificate in person. You can also order online at www.vitalchek.com (additional fees may apply for online orders).

Social Security Card Information

If you would like a social security card, check the appropriate box on the Birth Certificate Worksheet. It will be ordered automatically for you. Look for delivery of the card to your home address in about 6 weeks. There is no charge for this service.
