Access your minors' medical records

Intermountain Health is refreshing My Health+ to look more like our new brand, and we’ve made it easier to find by simply calling it Intermountain Health’s patient portal.

Gain access to your minors' medical records through the Patient Portal.

These instructions are for requesting access via the My Health+ mobile app. You can also request access through the web version of My Health+. The functionality is the same, but the visual prompts will look different.

Step 1: Log into the Patient Portal

Existing Users: You can log in using your existing My Health (Intermountain's legacy health portal), SelectHealth, or My Health+ username and password.

New Users: If you don't have an existing username and password you can create an Intermountain Health patient portal account

The Intermountain Health Patient Portal

The Intermountain Health patient portal is available as a mobile app and a web experience. However you choose to access it, the patient portal is the right place to manage your health information on the go.

If you’re new to the patient portal, you’ll be pleasantly surprised how simple it is to access your health records, message your provider, schedule an appointment, pay your bill, and so much more.

Download the app or go to the web experience to get started today.

Step 2: Fill out the request form

Once logged in, click on your name and account management. 

Click "People I have access to" then click "Request access to another account"

Follow prompts to fill out the form and submit

Our team will review and notify you via email when it's complete. (Within 2 Business days).

Step 3: Sign in and refresh users list

Log into the patient portal if you've been granted access to the person's account in the notification. 

Click on Your Name / Refresh User list and follow the prompts

Click Continue with Refresh. This will log you out and finish syncing your account.

Frequently Asked Questions

Frequently asked questions concerning gaining access to your minors medical records

If you are experiencing issues while self-requesting access to your minor’s medical information, please contact 800-442-5502 to help complete the process.

While we can grant your login with access to multiple medical records, we will still need your consent after the linkage is complete to access this information inside the patient portal. This linkage only needs to be done when you add new people to your account.

Generally, HIPAA regulations require an adult patient to complete a written authorization form or provide other legal documentation for granting an adult’s access to another adult’s medical records. As such, access to another adult’s account can be done in-person at your Intermountain clinic. Both you and the adult patient whose medical records you want access to must be present with appropriate legal identification such as driver’s license, government-issued ID card, or passport.

After completing the details for your first minor, there will be an option within the form to “Add Another Minor” before submitting your request. If you’d like to add multiple minors, please click “Add Another Minor” and continue completion of all the required information before final submission.

Within the login screen, you can select “Forgot Username” or “Forgot Password.” You will need the e-mail address that you believe is linked to your account in order to retrieve your username. If you are still unable to log in, please call 800-442-5502.
