List of Intermountain OHCA Members and Activities:
1. IHC Health Services, Inc., Intermountain Life and Health Benefit Plan, SelectHealth, Inc., and the medical staff for IHC Health Services, Inc. (Medical Staff OHCA).
The Medical Staff OHCA Members may exchange PHI to the extent otherwise authorized or permitted by federal and state law and for joint health care activities.
2. IHC Health Services, Inc., Intermountain Life and Health Benefit Plan, SelectHealth, Inc., and the University of Utah Medical Group Pediatric Sub-specialists (PSS OHCA)
- The PSS OHCA Members may exchange PHI without your authorization, and only to the extent otherwise authorized or permitted by federal and state law and for certain joint activities to support the delivery and management of high quality, innovative, and cost effective care.
- These activities include:
- Facilitating improved coordination of the healthcare you receive through population health management
- Evaluating and measuring the quality and cost outcomes of care protocols
- Informing clinical programs and assisting in the development of new clinical standards or processes
- Providing information to Intermountain’s care management resources to design and improve care coordination processes
- Creating analytics and data for healthcare providers, concerning the cost and utilization of their patients, to close care gaps and improve care
3. IHC Health Services, Inc., Intermountain Life and Benefit Plan, and SelectHealth, Inc. (Health Services/SelectHealth OHCA)
- The Health Service/SelectHealth OHCA Members may exchange PHI without your authorization, and only to the extent otherwise authorized or permitted by federal and state law and for certain joint activities to support the delivery and management of high quality, innovative, and cost effective care.
- These activities include:
- Facilitating improved coordination of the healthcare you receive through population health management
