Utah law prohibits hormone therapy and puberty blockers as treatment for gender dysphoria for anyone under 18 who did not have a documented gender dysphoria diagnosis before January 28, 2023.

Surgical procedures

Intermountain Health works with providers who offer a variety of gender-affirming care services in Utah including hormone treatments and surgery for adults.

Our plastic surgery team offers gender-affirming top surgery that reduces the size of the chest and nipples to give it a more preferred appearance. Our specially trained care team can give you letters of support, education, insurance coordination, and surgical intervention. Our multidisciplinary care team follows the World Professional Association for Transgender Health (WPATH) standards of care.

Facial surgery includes procedures to bone, skin, and soft tissue of the face. Intermountain Health’s plastic surgery team does not offer these procedures currently but plans to in the future. Your care team can help navigate your care, including offering referrals for outside providers.

A hysterectomy is a surgical procedure to remove the uterus. An oophorectomy is a surgical procedure to remove the ovaries. These are often done at the same time. This will impact a person’s fertility and you should talk with a gynecologist and/or a fertility specialist before having this surgery. Both are required before undergoing gender-affirming phalloplasty.

This is an outpatient surgery to remove the testicles and it will impact a person’s fertility. You should talk to a urologist and/or a fertility specialist before having this surgery. This surgery can be done prior to or at the same time as a vaginoplasty or vulvoplasty when those procedures are part of your treatment goals.

Vaginoplasty is a surgery that’s done to make a vulva and vaginal canal from your existing tissue. This can be done in a lot of ways. A vulvoplasty is a surgery done to create a vulva without a full depth vaginal canal. You should talk to your referring provider, therapist, and surgeon about these options, along with advantages and disadvantages.

Intermountain Health does not offer this surgery at our facilities for gender dysphoria; however, your care teams will offer referrals to outside providers.

This is a surgery which is done in at least three stages to create a penis and a scrotum from existing tissue. The goal of this surgery is to create masculine presenting genitals, urinate while standing, and have penetrative sex. A skin graft is needed for this procedure and can be taken from different areas of your body. Usually, it’s from the underside of your forearm, or the upper side of your thigh. You’ll talk about this choice with your surgeon during the consultation appointment.

Intermountain Health does not offer this surgery at our facilities for gender dysphoria; however, your care teams will offer referrals to outside providers.

Pre- and post-surgical care

Before your surgical consultation, you’ll need to meet with a social worker to help with care navigation.

During the consultation with your surgeon, a complete health assessment will be done to make sure you are healthy enough for the surgery. This visit also includes a physical exam. A breast tissue exam will be done as one of many breast cancer screening tools. A few pre-op conditions will be discussed including weight optimization, smoking cessation, hair removal, breast cancer screening, imaging (MRI, CT scan, mammogram, ultrasound if necessary), and others when indicated.

After surgery you will meet with the surgical team several times to ensure proper healing and recovery. This will include examining incisions, removing drains (if applicable), checking for signs of infection and bruising, and other factors unique to each surgical procedure. The post-op healing time varies for each procedure, and you will talk about it with your surgeon during the consultation and throughout the healing and recovery time.
