Transfer services

Our transfer specialists facilitate all types of patient transfers into all Intermountain Healthcare facilities. They are experienced in Stroke, STEMI, and Trauma Protocols to ensure patients are transported quickly and safety to the right physician at the right hospital every time.

When calling, please have the following information available:

• Patient name, gender, and DOB
• Diagnosis/Condition
• Contact number for the physician requesting the transfer (no pagers)
• Name of sending facility/unit and call back number
• Reason for transferring patient:
  • Higher level of care
  • Service not provided at sending facility
  • Continuity of care

The Patient Transfer and Access Center at Intermountain Healthcare is staffed 24/7 by Registered Nurses and is the single point of contact for all patient transfers into the Intermountain Healthcare system, including Primary Children’s Hospital. Both referring and accepting physicians can make one call to the Intermountain Transfer Center, and we will do the rest.

Our transfer specialists will arrange patient transfer, bed placement, and appropriate transport into all Intermountain Healthcare facilities. They are experienced in Stroke, STEMI, and Trauma Protocols to ensure patients are transported quickly and safety to the right physician, at the right hospital, every time.

The service will also connect providers with physician consultants and Intermountain Connect Care Pro TeleHealth services.
