Once you complete the intake form, a clinical pharmacist will contact you and determine qualification.

This service is a great option if:

  • You are experiencing signs and symptoms of athlete’s foot localized to your feet (i.e., redness, itching, burning, flaking)
  • You are 18 years of age or older.
  • You don’t have a fever (greater than 100.4°F), diabetes, or have an immunocompromising condition (i.e., HIV, cancer)

If you do not qualify, the clinical pharmacist can assist with a referral to an Intermountain provider for evaluation.

  • Ability to upload 2 pictures of your feet where symptoms are occurring
You can expect to be contacted by the clinical pharmacist within a business day (between 8am and 5pm MST) following submission of the intake form. Generally, the medication will be delivered to your home within 3-5 business days from the Intermountain Home Delivery Pharmacy. Shipping is free. The clinical pharmacist will work with you to identify a different arrangement if a more rapid turnaround is needed.

The cost of the service is a $35 consultation fee plus the cost of the medication after any insurance coverage. If you do not qualify, you will not be charged for the service.

Potential medication costs:

  • $5-$20 for over-the-counter (OTC) treatment options
  • $0-$30 for prescription treatment options

Complete the online intake form so a licensed pharmacist can determine the right treatment for you. A pharmacist will call you within 24–48 hours to discuss treatment options.

We’ll make up to two phone calls and one email if we can’t reach you right away by phone. Usually, we cannot provide a prescription without speaking with you first.

Refunds are not given if we cannot reach you via phone or email. Bank payments cannot be refunded.

If too much time passes after you send your form and we can’t reach you, you may need to fill out a new one so we have your updated health information.
