Platte Valley Hospital offers CHIP grants

Platte Valley Hospital provides grant opportunities through the Community Health Investment Program (CHIP). The grant is meant to assist organizations with programs that improve the health of our community, especially for our low-income residents. Your grant application must show how you will impact the health of your target population. To be considered for a CHIP grant, your organization must meet most if not all, of these criteria:

  • Support the mission of Platte Valley Hospital, “Helping people live the healthiest lives possible”
  • Serve a demonstrated need in the community
  • Proposals that involve collaboration with other community providers and organizations that are directly or indirectly involved with community health concerns will receive priority
  • Have a reasonable financial request.

Applicants for CHIP grants must meet the following minimum requirements:

  1. The organization must have a 501(c)3 letter of determination or a government/ quasi- governmental unit and their status must be currently in good standing.
  2. The organization must operate under written Articles of Incorporation, By-laws or other written documents or statutes which define the organization’s purposes, membership, management, and operation.
  3. The organization must operate on a non-discriminatory basis in employment, recruitment of volunteers and delivery of services.
  4. The organization must demonstrate effective program performance and financial responsibility and accountability by submitting three years of financial audits or financial statements.
  5. The population being served by the grant funds must be located within the geographic area served by Platte Valley Medical Center (Brighton, Commerce City, Ft. Lupton, Henderson, Lochbuie, Keenesburg, Hudson, Wattenburg).
  6. The organization must complete a vendor application and be approved as a vendor through Intermountain Health before funds are distributed.
  7. Grant recipients shall submit a final report to the CHIP committee indicating the actual use of the grant within 60 days of receiving the grant or no later than one year after receiving grant funds.

The Committee will make decisions on grant requests at regularly scheduled meetings, which occur quarterly in February, May, August and November.

Applications must be received by the 15th of the month prior to the next scheduled meeting.
