Keeping Patients Closer to Home

When a patient in a rural community needs to receive specialty care, the patient is often transferred to a facility hundreds of miles away from their families and communities – the support system they need most. Meanwhile, rural hospitals lose much-needed revenue, and communities face the very real possibility of losing their critical access hospital. Intermountain TeleHealth is reversing this trend, keeping patients as close to home as possible. Allowing them to receive high-quality specialty care from Intermountain’s world-class experts no matter where they are.

Our Goals

Intermountain TeleHealth supports keeping patients closer to home by bringing specialty services to communities that a patient would otherwise have to travel from to receive specialty care. In turn, TeleHealth assists in keeping revenues in local community facilities.

1. Keep patients in their own community

Intermountain Healthcare is internationally recognized for its care process models. Partnering with Intermountain TeleHealth brings Intermountain’s experience and best practice models to administrators, providers and caregivers at your facility.

2. Create integrated care management models

Partnership in the delivery of care is essential to the success of telehealth. A specialist on the screen must have knowledgeable clinical staff on-site to deliver care. Our team works to not only build confidence with your team, but to create and maintain relationships to where Intermountain caregivers are a true extension of your team.

3. Develop clinical and operational partnerships

Collaboration with Intermountain’s specialists inherently becomes a training ground for higher-level of care in your facility. Our specialists share what they know, walk your caregivers through recommended treatments and ultimately help them feel confident in keeping higher-acuity patients in your facility – closer to home.

4. Build local skills and complex acuity care confidence

Intermountain has seen telehealth patients have outcomes equal to that of a patient receiving in-person care at one of our facilities. This is done through partnered implementation of and adherence to Intermountain’s best practice standards of care.

5. Implement standards of care and adherence practices

Intermountain TeleHealth expands the reach of a specialist to multiple facilities where on-site caregivers are able to deliver in-person care with the oversight and experience of a specialty trained provider. Rethinking care in this way allows patients to stay closer to home, local facilities to provide a higher-level of care and revenues to stay at the local facility.

6. Redesign clinical practices to blend digital options and in-person services

Partnering with Intermountain TeleHealth

We strive to help our partners improve access, lower costs, retain revenue and improve patient experience through:

  • Improved speed-to-treatment
  • Retaining higher acuity patients
  • Decreased length of stay
  • Reduced mortality rates

Partners will have access to Intermountain’s Care Process Models, build collaborative relationships with and learn from best-in-class specialists, and gain the benefits of partnering with a large healthcare system while maintaining independence. Clinical Coordination Teams, including intensivists and hospitalists, consult with onsite providers on the need for a transfer, optimize a patient for safe transport, and coordinate with the other Telehealth Support Services, including Transfer Center, Patient Placement, and Life Flight.
